Carl D. Perkins Application
To apply for assistance, you need to complete an application and provide copies of the listed documents on the application. Please submit your Public Assistance Award Letter for any of these types of assistance that apply to you, such as WIC, Food Stamps, Medicaid, TANF, and SSI. If you are applying for daycare assistance, please also submit a Birth Certificate copy for each child in order to be considered for assistance from the grant. Applications and further information can be found at the Administration Buildings of the following campuses or by downloading below.Completed applications and documents can be scanned and emailed to perkins@swtjc.edu.
All scanned documents should have any social security number(s) removed before submission.

Carl D. Perkins Qualifications
Qualified career and technical education students are eligible to receive funds for student support services under public Law 101-392. To help determine your eligibility, please complete this form. Allocations will be determined by the number of participants and the availability of funds.
You may qualify for assistance from this grant if you are enrolled in a Technical Program and can show economic need.
- Individual with Disabilities
- Individual from economically disadvantaged family, including low-income youth and adults
- Individual preparing for non-traditional field
- Out-of-workforce individual
- English Learner
- Homeless individual
- Youth who is in, or has aged out of the foster care system
- Youth with a parent who is a member of the armed forces and is on active duty
